Chapter 4 Personal Hygiene Section 1 Oral Hygiene
Objectives To provide increased comfort To allow the nurse to assess the client’s oral health status, knowledge, and routines of oral care. To provide client teaching when appropriate. To remove unpleasant tastes and odors from the oral cavity. To decrease the possibility of irritation or infection of the oral cavity. Preparation Assessment of client 1. Choice of mouthwash Implementation Providing oral hygiene for normal client(learn by self) Providing oral hygiene for special client Equipment Middle-sized therapeutic basin中治療盤(pán): Flashlight手電筒, swab棉簽, lubricant for lips潤(rùn)滑劑, gentian violet紫藥水, mouth-gag開(kāi)口器, tongue forceps舌鉗 (if necessary), protective pad治療巾, kidney basin彎盤(pán) 2 (tong招生簡(jiǎn)章ue blades壓舌板, forceps鑷子, hemostatic forceps止血鉗, gauze紗布, normal saline cotton ball26~30生理鹽水棉球 [water in cotton ball should be moderate to prevent aspiration吸氣. Procedure Before the procedure there are some key points that should be known: 1. gather equipment. 2. Identify the client. 3. Explain the rationale for the procedure to the client. 4. if possible, position client on side in a lateral position or supine position, turn client’s head to the side. Allows fluid to drain out of mouth and thus prevents aspiration. 5. place underpad on client’s chest and near by chin. 6. inspect surface of mouth for any abnormalities. 7. put kidney basin under chin. 8. brushing order. 9. assist client to rinse mouth and expectorate into kidney basin. 10. remove basin and wipe off client’s mouth with gauze. 11. lubricate client’s lips with petroleum jelly or swab wound with gentian violet if necessary. 12. dispose of equipment. 13. return client to comfortable position. Clear up client’s bed. 14. place equipment in appropriate receptacle. Precaution 1. lightly brush. 2. brushing thoroughly and having a rational brushing order. 3. water in cotton ball should be moderate to prevent aspiration. 4. assess the client’s oral health status to see if there are dentures, oral ulcer or infection, and deal with it accordingly. 5. provide oral care frequently.
