【摘要】  目的:檢測乳腺癌雌激素受體α(estrogen receptorα, ERα)基因啟動子區(qū)CpG島甲基化狀態(tài),探討乳腺癌ERα啟動子甲基化與其蛋白表達及臨床病理的關(guān)系。方法 :采用免疫組化EnVisionTM二步法檢測20例正常乳腺組織、44例乳腺癌組織ERα基因的表達,同時運用甲基化特異性PCR(Methylation-Specific PCR,MSP)及測序法分別檢測ERα基因啟動子A區(qū)和B區(qū)CpG島的甲基化狀態(tài)。結(jié)果:32例ERα陽性乳腺癌組織中,ERα啟動子A區(qū)、B區(qū)甲基化率分別為28.2%和18.8%。12例ERα陰性乳腺癌組織中,ERα啟動子A區(qū)、B區(qū)甲基化率分別為66.7%和58.3%。ERα陽性乳腺癌組、ERα陰性乳腺癌組、乳腺癌組的ERα啟動子A、B區(qū)甲基化率均較正常組顯著增高(P <0.05);ERα陰性乳腺癌組的ERα啟動子A、B區(qū)甲基化率較ERα陽性乳腺癌組均顯著增高(P <0.05)。乳腺癌組織ERα表達與啟動子區(qū)甲基化之間呈顯著負(fù)相關(guān)。結(jié)論:乳腺癌組織ERα基因表達沉默與其啟動子A區(qū)、B區(qū)CpG島甲基化相關(guān),后者有可能成為乳腺癌預(yù)后不良的分子檢測指標(biāo)。

    Abstract:  Objective: To detect the methylation of CpG island in estrogen receptor alpha (ERα) gene promotor in breast cancer, and study the relationship among ERα gene promotor methylation,ERα expression,and the clinicopathological characteristics in breast cancer. Methods: The EnVisionTM two-step immunohistochemistry method was used to detect ERα gene expression in 20 cases of normal breast tissues and 44 cases of breast cancer tissues. The methylation of CpG island in ERα gene promotor region A and B was respectively detected using methylation-specific PCR (MSP) and sequencing. Results: In 32 cases of ERα positive breast cancer tissues, the methylation rates of ERα promotor region A and B were 28.2% and 18.8%,respectively; while in 12 cases of ERα negative breast cancer tissues, the methylation rates of ERα promotor region A and B were 66.7% and 58.3%,respectively. The methylation rates of ERα promotor region A and B in ERα negative breast cancer tissues, ERα positive breast cancer tissues and breast cancer tissues were significantly higher than that in normal breast tissues (P <0.05). The methylation rates of ERα negative breast cancer tissues were significantly higher than that of ERα positive cases (P <0.05). A negative relationship was indicated between ERα expression and the promotor methylation in breast cancer. Conclusion: The silencing of ERα gene expression is suggested to be significantly related with the methylation of CpG island in the ERα promotor region in breast cancer. The methylation of CpG island may become the molecular marker indicating bad prognosis of breast cancer.
